**Fellow-In-Training, Student, and Allied Health registrations require proof of status: a letter on department letterhead confirming your status or a copy of your valid University identification card is required. You will be asked to upload a document in PDF, Word or an image format while registering online.

*Delegates who not wish to attend the Welcome Reception will benefit from a reduced registration fee.
Payment:
Registration payment can be made by credit card (Visa, MasterCard, American Express, or Discover) or by wire transfer. Registrant is responsible for all fees associated with a wire transfer. Please send a confirmation of wire transfer by email to symposium@Showcare.com or by fax at +1 (514) 289 9844. Please include at least one of the following with your wire transfer:
- The name of the registered delegate
- Delegate email address
- Registration ID
Cancellation Policy:
Please send cancellations by e-mail to symposium@Showcare.com or by fax to +1 (514) 380 5383.
Cancellations will be processed according to the following schedule:
Before/on 31 July 2015: Full refund less $85 USD administrative charge
1 August - 30 September 2015: 50% refund
After 30 September 2015: No refund
On-site Registration:
4 December 2015: 15:00 - 19:00
5 December 2015: 07:00 - 18:00
6 December 2015: 07:00 - 15:00
On-site registration hours are subject to change.
Substitution Policy:
No substitutions are allowed. Individuals will have to cancel the original registration and the new person will have to create a new registration. Within groups, however, you will be able to do substitutions at any time up until the regular registration rate deadline (30 September 2015). After this date, you will be charged an $85 USD administrative charge per substitution/name change.
Wire Processing Fee:
A $50.00 USD processing fee will be applied to all bank wire transactions.
Processing Fee:
A 3% credit card processing fee will be added to all credit card transactions of $10,000 USD or more.
